For Hospital and Health Teams

NDIS Support Ready for Discharge Day

Refer a patient with NDIS funding and we'll call you back the same business day. From there, we meet the person on the ward and plan their support with your team. Our support workers live in Melbourne's west and north, so the person goes home to someone who knows their suburb. Support is in place the day the person leaves hospital, so the discharge isn't held up waiting on us.

  • Same-day reply
  • Taking urgent referrals now
  • Local support workers

Ready When Your Patient Is

Set Up While You Assess

We match local support workers to the person and get the service agreement signed while your team finishes its assessments.

Tell us the discharge date and we'll have support ready for it. If the date changes, we change with it.

A Clean Handover

We work directly with your MDT and keep the family, support coordinator and NDIS health liaison officer updated at every step.
